Have you ever wondered why a lot of non-US IMGs unable to secure any interviews despite their strong qualifications?

The reason is because there is an "X" factor in your application:

Letter of recommendations

Letter of Recommendation is the "X" factor of your applications because it's the part of your application that you have no full control over what's being said about you, yet, it's one of the most important factor that programs used to decide who to interview.

The way to secure strong LoR is by doing USCE

So now, we have a big problem to face...

#Standing Out and Making Strong Impression During USCE is Very Hard for Non-US IMG

Non-US IMG faced challenges in the US due to language and cultural barriers. What you consider to be polite, might sound rude to americans. The cultural mismatch easily lead to misunderstandings. in addition, you are not familiar with the US healthcare system:

Do you know the difference between a Social Worker and Case Manager?

Do you know when to consult them?

Do you know the discharge facilities in the US? we have long term care facility, assisted living facility, acute rehab, subacute rehab, home with VNA, and many more.

Do you know how the insurance works?

Do you know we have clinical pharmacist, respiratory therapist, nutritionist, picc line nurse, wound nurse, LNA, and many more staffs in the hospital. do you know their roles? do you know how to work with them?

Do you know how to work with EMR?

Have you ever review US medical charts? they are all full of unfamiliar terminologies, acronyms and brand names

Remember that the rotation usually last 4 weeks max (some are 2 weeks) and you only have that much time to adapt to the system, stand out, impress the attending, make connection with people around you.

The way I see it, this process is so unfair to Non-US IMG. In order to secure strong LoR to help you match, we are forced to adapt and impress people both at the same time. It feels like a losing battle to begin with. I know exactly what it feels like

Because I was YOU years ago!

I am non-US IMG who has struggled with language, cultural differences, and US medical system.

Let me share you my USCE experience:

I went to my first inpatient USCE with full of confidence. Because I scored 258 on my step 1, 261 on my Step 2 CK, actively tutoring students at that time, I am at the peak of my medical knowledge! I feel I can handle anything! Guess what happen in the first week...

No one taught me how to operate and use EMR, I spend hours prior to round accomplishing nothing, I barely understand half of the notes, there were gigantic amount of information, unfamiliar medical terminologies, acronyms, I can't understand what is going on with the patient!

The interns are all busy, I was shy and not sure how I should approach people in this new environment, no one was able to teach and guide me how to review the chart, how to present case properly, getting familiar with these different terms, the role of each people in the hospital, after the end of 1st week, this is what the senior resident said to me...

Hey! Why are you slacking off?

This is literally what he said to me. He said this to me because I was going around accomplishing nothing. because I confused, and not familiar with the system. And guess what? I can't respond to him, because my English was SO BAD! I was unable to express myself and explain! then, these were the next words that he said to me:

Aren't you supposed to be a graduated doctor? Even an MS-3 (3rd year medical student) can handle more patients than you!

Again, I have no way to respond to this statement. Also I figured that If I responded to this, it would just become an excuse. That is not something that I want to do. So I accepted it, and find ways to improve MYSELF. Testing different ways to review the chart so I can be efficient, finding what are the sequence of chart review that would make sense, paying more attention during rounds of all these different people and roles in the hospital, googling for the new terms and ask these one by one to get more familiar with them.

Finally, on week 4 I was able to pull off great presentation, and secure the LoR from the very LAST ATTENDING (the attending changed every week). So I made it, but it was ROUGH.

If only there is a way for me to be more prepared toward USCE, I would have secure More LoR.

I don't want you to experience what I had. I am passionate in helping Non-US IMG to Match, so I am actively looking for solutions. And guess what,

I HAVE FOUND THE SOLUTION.

Imagine, with this solution, each of you can be prepared for USCE, take control of how people perceive about you, and secure strong Letter of Recommendations

INTRODUCING

I highly recommend you watch the very first video above so you can dive into the whole course of USCE Prep Course, everything that I got to offer! Below is the quick summary of USCE Prep Course features!

USCE Prep Course brings together over 7 years of my combined experience and knowledge as an internal medicine resident and attending physician

The Main Goal of This Course is to Help Non-US IMG Adapt to US Healthcare System and Environment, Ultimately Securing Strong Letter of Recommendations From Your USCE Rotations

"In simple terms, by fully completing this course, you will attain the communication skills, medical knowledge, and confidence needed to work effectively in the US healthcare system—equivalent to that of a First-Year Intern."

USCE Prep Course includes Telerotation Observership Simulation which is the First of its kind!

The components of tele-rotation involves observing interaction between doctor and patient, didactics, patient chart review, case discussion and analysis, constructive feedback on your documentation, case presentation, and clinical assessment and decision making.

you will get all of the above in USCE Prep Course

(P.S: in the picture below you will see the name USCE Masterclass which is the previous name of USCE Prep Course)

Here let me break it down further for you, you will have access to:

1. Chart review. You will receive access to USCE Prep Course EMR Practice (see sneak peek below) which contains US medical notes and practice reviewing them, complete with their workups including labs, imaging, EKG, biopsy, everything that you need, just like in real life. (Please note that these are not real patient medical notes, these are dummy notes that are inspired by real cases for learning. Therefore, there will be no real medical pictures available. But even if it's not real patient, you will get the same educational nuance and real-life patient difficulty.

2. Listening to audio conversation between patient and doctor. Yes, you will receive audio recording for some of the cases that you review (Another note, these audio recordings are not real conversation that taken place inside the clinic, but recreated/simulated by using hired actors to match the story with the dummy cases above, the quality of these simulations are very high that resembles real-life interaction)

3. Case presentation. You can practice presenting the case by yourself using the information provided.

4. Case discussions and analysis. Quizzes are provided for each case to help optimize learning process. there will be audio recording of my clinical thought processes handling the cases, and short didactic is provided related to the cases

This is something that no one has ever made before and can't find anywhere else!

Testimonials

I certainly would recommend the masterclass to all non-US IMGs because there’s so much information unpacked in the masterclass that would help any IMG learn about the system before going into residency. It even enhances our confidence to answer questions in the interviews we’ll be giving!

I am deeply grateful for the opportunity to be a part of your masterclass. Thank you for all the efforts you have put into it, it’s truly inspiring and sincerely appreciated.

- Aayushi

I am writing to express my sincere gratitude for the exceptional USCE modules you have developed. As a Non-US IMG, I found the content to be highly engaging, informative, and valuable.

Your thorough and thoughtful approach to each module was evident, and I appreciated the clarity with which the topics were presented. The ability to cater to the unique needs and perspectives of Non-US IMGs is a testament to your expertise and commitment to providing a comprehensive educational experience.

Thank you for your encouragement and for fostering an atmosphere of active participation throughout the course. Your efforts have truly enriched my understanding of the USCE process, and I am grateful for the opportunity to be a part of this valuable learning experience.

I learned about the course through your YouTube channel, where the clear presentation and unique perspective in your videos motivated me to join this course.

Dr. Haryadi’s USCE Masterclass has been an incredible resource, especially for someone new to the US healthcare system like me. The neuro-psychiatric cases stood out as particularly informative. These cases, including dementia and severe mental illness, highlighted the importance of understanding multidisciplinary care. I learned about essential components like CIWA assessments, which are crucial for managing such patients in an inpatient setting. This knowledge is invaluable as it helps bridge the gap between psychiatry and internal medicine, preparing me for rotations and residency without surprises.

The lecture delivery was excellent—clear, concise, and perfectly paced. I truly appreciated the simplicity with which the information was presented, making even complex topics approachable. The Q&A session was particularly helpful in addressing my questions, and I felt fully supported throughout.

A suggestion for future modules would be to include more cases involving multidisciplinary care or basic components of other specialties relevant to internal medicine. Additionally, a video walkthrough of the US healthcare system PDF would be highly beneficial. Adding visuals like images of a Methadone clinic could further enhance understanding and retention.

Overall, this webinar was exactly what I was looking for. Dr. Haryadi’s ability to consolidate years of knowledge into a single course is remarkable. I’m grateful for the effort and dedication he’s put into making this learning experience so impactful."

Dr. Haryadi’s USCE Masterclass is an amazing resource for IMGs navigating the complexities of the US healthcare system. Module 2 was incredibly informative and introduced many new concepts for me. Among the seven cases discussed, I found the one about undocumented immigrants to be the most insightful, especially regarding the financial aspects of their care. It provided a perspective I hadn’t encountered before and highlighted the nuances of handling such cases.

The lessons delivered a wealth of information on how cases are managed, offering practical, real-world insights. I’ve never come across any other course or resource that explains these topics with such depth and clarity. The clear and concise presentation made complex ideas accessible, even for someone unfamiliar with the US healthcare system.

One area for improvement could be the pacing, which felt slightly fast for someone like me who is still learning the hospital dynamics in the US. Adding a video walkthrough of the US healthcare system PDF, with visuals and examples, would be extremely helpful.

to make it a no-brainer deal for you I’m giving you a 30 day 100% money-back guarantee with your purchase of USCE Prep Course today.

That’s right: Go through the course, watch everything, do the cases, and if you don’t feel that it's helpful for you in anyway, in the first 30 days of your purchase, send me an email and I’ll refund you immediately. No questions asked, no hard feelings.

Let me give you another bonus with USCE Prep Course, since a lot of people has been asking me for this one:

You can get a chance to do 1 week rotation with me in my outpatient clinic in the office

Why only 1 week?

It's simple, my employer won't allow me to do more, I wish I can provide more...

However, on the positive side, it actually allows more students to come as I would like to provide exclusive 1:1 interaction, rather than having 6-7 different students come at once.

During my rotation, you will have a chance to:

  1. Observed direct application of USCE Prep Course. You will see how I applied everything that I taught you in USCE prep course: cross cultural communication, fundamental principles of patient interaction in the US, how to think like an attending, and so much more
  2. Learn to chart review using EPIC EMR directly, under my supervision
  3. Actively engaged in the clinical activities: you will have a chance to take additional history from the patient (and only if patient allows it, you can have a chance to do physical exam as well, under my supervision). Your medical knowledge will continuously be challenged, and we will go through and analyze the cases together.

The space are limited, as we only have 52 weeks per year and I don't work every week.

Note that you have to complete all the USCE Prep Course lessons and pass the quiz, prior to the rotation.

Once you purchase the USCE Prep Course, the earliest available date for booking will be 30 days from the date of purchase

I'm so glad I got to take Dr. Haryadi's Masterclass and do an observership under his supervision.

The Masterclass was incredibly helpful for me as an IMG, especially since I knew so little about the U.S. healthcare system. It really opened my eyes. Dr. Haryadi walked us through how to work with the EMR, covering all the important parts through chart reviews. The course also explains the U.S. health system and the resources we have as PCPs or hospitalists when dealing with different cases. Plus, we learned about the communication and interpersonal skills that are essential for IMGs as they transition to U.S. health system.

My observership at Wentworth Health Partners Internal Medicine in Dover, NH, under Dr. Haryadi’s supervision in August 2024 was also amazing. I got to observe and participate in various clinical activities,  The hands-on experience with the EPIC EMR system was super valuable, and I gained a deeper understanding of the U.S. healthcare system in practice.

Overall, I highly recommend both the Masterclass and the observership with Dr. Haryadi to all my fellow IMG friends!

In addition, during my interview, one of the faculties specifically mentioned at the very beginning that my LORs were very strong, and that they highlighted my compassion for care. He then mentioned that we are exactly looking for compassionate
residents who can connect with our patients. So, thank you so much for
your strong LoR!"

Yes, you read it correctly, you can get Strong Letter of Recommendation by going through USCE Prep Course

A lot of people ask, is 1 week rotation enough for me to get strong, personalized LoR?

ABSOLUTELY!

Think about it this way:

USCE Prep Course contains my 7+ years combined experience as Internal medicine resident+ attending. You will have chance to review so many patients, you will receive feedback and learn the cases directly from me.

You are basically going through the rotation with me

But, I don't want it sounds like you purchase the letter of recommendation from me ( I am not that low)

Letter of recommendation is still based on merit, so you need to show me that you worth to get my letter of recommendation

So this is how you can get my letter of recommendations:

1. Complete all the lessons (100% completion rate)

2. You need to pass all quizzes available in each module. There will be patients for you to practice, and there will be set of patients for the test. You will be tested with 3 patient cases and you will need to get >60% correct on all the quizzes. The question format would be similar with USMLE Step 2 CK, with the exception that the patient information will be obtained from the chart review.

3. Once you completed number 1 and 2, you will schedule an in-person rotation with me

Once all of the above completed, you will receive personalized, STRONG, letter of recommendations. This is pretty much guaranteed provided you completed the requirements above!

Please note that rotation is first come first serve.

USCE Prep Course is for you if:

You are non-US IMG looking for USCE, Lor, and wants to match

it would be the best fit if you are looking to match to internal medicine (family medicine are welcome, though all cases are adults and no obgyn cases). people that want to match to other specialties will still benefit from all the modules with the exception of USCE Prep course EMR practice and rotation.

USCE Prep Course is not for you if:

You don't need USCE or LoR to match

FAQs and additional information

1  Will the letter of recommendations be written in hospital letterhead?

I don't think having a hospital letterhead will really help you to secure interview or match in any way. Please note that there are 2 important things about LoR. First, are the credentials. I don't work in a random private clinic. I am fortunate enough to be a part of highly reputable institution, Mass General Brigham. This provides strong credibility in your LoR. Secondly, the content of the LoR itself. Strong, personalized LoR is pretty much guaranteed if you have completed the requirements. Therefore, this LoR will become one of your key factors to secure interviews and match into
residency

2. I am not confident to pass the quiz since I have not done step 2 ck yet, If I fail the quiz, can I retake it?
Yes, you can retake it as many times as you want! though I will have to provide you different cases and quizzes. That is why I said LoR is pretty much guaranteed I am sure you will eventually pass if you study well.

3. Am I eligible if I have green card or US Citizen?
Yes, the only condition is that you must be non-US IMG

4. Accommodation for in-person rotation:

you can use airbnb or other websites if you want to get it cheaper, but the closest place is homewood suites, this hotel is walkable distance to my office. the cost is about $200 a night unfortunately.

5. What are the documentations needed for in person rotation?

all you need is covid vaccine records (no step 1 or step 2 ck score needed though highly recommended, or anything else). but surely you need to have a visa to be in the US. any visa is fine, you can also be a permanent resident or US citizen, as long as you are non-US IMG, I will gladly take you.

6. I am doing my USMLE Step 1 right now, should I purchase this after I completed Step 2 CK? in other words, when is the best time for me to start this course?

I would say the most ideal moment to start the course is after you have completed step 1 and studying for step 2 ck. Because at this time, you need to begin searching for USCE, and all US doctors begin their clinical rotation right after step 1 anyway. Sure, you can choose to wait until you completed step 2 ck, but remember the rotation is first come first serve, you would need to secure the LoR prior to September of that match cycle. You can only book the appointment with me after you have completed the course 100% and pass the quizzes.

7. How long does it take for me to complete the whole course?

I delivered the whole course during prelaunch live in 4 days, 3-4 hours each. Depend on your speed, you can complete 2 cases per day, that would be a total of 2 weeks. So I usually tell people that you might need a whole month to fully completed the course. again, it really depends on your schedule and your speed.

8. is the Letter of recommendation or this USCE experience will be valuable for me to put in ERAS CV since its only 1-week rotation?

This is one of the most common questions being asked. Although you do 1 week in person rotation with me, it is enough for me to get to know you and write you a strong letter of recommendations.

in addition, you actually have been learning under me for more than that. it typically takes about 1 month to complete the modules and the cases. so in the LoR, I would mention that you have been working with me for 1 month, not 1 week.

This actually means, you will receive a value of 1 month rotation with just only 1 week!

1 week staying in US for accomodation (airbnb or hotel) , travel, and food would have cost about an additional $500. You actually save so much money rotating with me by just doing 1 week in person and the rest as telerotation!
